Southwest community members will gain a comprehensive understanding of the preventative medical care experience through targeted health education information from trusted providers within the Chicago healthcare network. We aim to facilitate safe space to build trust between providers and community members within Southwest Chicago. Most importantly, our goal is to educate and empower community members to use existing health resources and to make better health decisions for themselves and their family members in the Southwest Chicago region.
